Request a new site

A site represents your workplace and must exist before you can add user accounts.

Choose a type of site to create:

Medical Practice/ Post Acute/ Billing/ Health Dept

Request access for a NEW BSW EpicCare Link account site group for your external medical practice/post-acute/billing/health dept. Please specify the "Facility Type" under the comments section on the Site request form. An option will appear to add all staff members to this request

Add a user to an existing site

If someone at your workplace already has access, your workplace has a site.

Choose a type of user to create:

Medical Practice - Non-Clinical

ONLY for existing external sites with BSW EpicCare Link accounts - Request access for a non-clinical staff member

Medical Practice - Clinical

ONLY for existing external sites with BSW EpicCare Link accounts - Request access for a clinical staff member

Provider

ONLY for existing external sites with BSW EpicCare Link accounts - Request access for a provider (MD, NP, etc)

Post-Acute Staff Member

ONLY for existing external sites with BSW EpicCare Link accounts - Request access for staff member at a post-acute facility (SNF, LTAC, etc)

Billing / Health Dept

ONLY for existing external sites with BSW EpicCare Link accounts - Request access for a Billing/Health Dept staff member

Research Monitor

Request access for a Research Monitor user

ONLY for existing external sites with BSW EpicCare Link accounts - Request access for a Baylor Scott & White DonJoy Global company staff member
